SUBJECT: RESOLUTION 2003-90; Authorizing CPGH Service Area Board to Award a Contract to PHRG (Public Health Resource Group) for the implementation of a Community Health Status Survey

The attached resolution authorizes Central Peninsula General Hospital Service Area Board to implement a Community Health Status Survey, in an amount not to exceed $49,600.

The purpose of this project is to perform a comprehensive survey that will remain a valid and reliable bench-marking tool; this tool should provide basic demographic information, health information (physical, emotional, mental and spiritual health), health service access and availability, and personality factors influencing health habits and behaviors.

On May 2, 2003, the Kenai Peninsula Borough solicited proposals for this project on behalf of CPGH Service Area Board. Four proposals were received on June 12, 2003 and were reviewed and evaluated by CPGH Service Area Board members. PHRG received the highest score, as per the enclosed evaluation summary.
